The Be the Light: Hope & Healing Program respects and protects your privacy.
Information submitted through this form will only be used to determine eligibility for counseling assistance and to coordinate a referral to a licensed mental health provider.
Be the Light does not provide clinical services and will not maintain therapy records. Once referred, all clinical services and health information will be managed by the licensed provider in accordance with HIPAA privacy regulations.
Be the Light will only receive general program reporting data such as number of individuals served and average sessions utilized.
